Commercial Office
If your business will be located in a commercially zoned facility
- Business Tax Receipt Application
- Occupancy Use & Zoning Review Form (newly constructed, change of use and adding additional services only)
Additional documentation needed (if applicable)
- Articles of Incorporation and/or Fictitious Name Registration
- State/Professional License
- Valid Identification
- Payment for Business Tax Receipt
- Payment for Occupancy Use & Zoning Review Fee (if applicable)
Home-Based (Residential) Office
If your business will be located in your residenceAdditional documentation needed (if applicable)
- Articles of Incorporation and/or Fictitious Name Registration
- State/Professional License
- Valid Identification
- Payment for Business Tax Receipt
Payment is due when completed application is submitted. Cash, Check, Money Order, Mastercard, American Express and VISA are accepted. After application has been reviewed and approved, the business tax receipt will be mailed. Contact our office for fees
